Building a realistic regulatory roadmap: milestones, resources, sequencing

The critical path of an MDR certification almost always runs through clinical data. Waiting for its completion to start the QMS adds 6 to 18 months to the schedule, while a 20 to 30 percent margin remains prudent.

A regulatory roadmap is not an ordinary project plan. Timelines depend on third parties — notified bodies, study providers, competent authorities — whose responsiveness cannot be controlled. A realistic roadmap incorporates these dependencies and builds activities around them, not in spite of them.

The structuring milestones of an MDR certification project

Milestone 1: MDR classification confirmed. This is the starting point. Until classification is confirmed, the full programme remains undefined.

Milestone 2: notified body selected and contracted. This milestone takes longer than expected — selection, comparison of offers, contract negotiation, signature. For class IIa and above, allow 4 to 8 weeks between first contact and signature.

Milestone 3: critical gaps identified and study programme defined. What data is missing? Which studies are needed? What is the timeline for each study? This milestone depends on the prior gap analysis and shapes the overall schedule.

Milestone 4: sufficient clinical data available. This is often the longest milestone. A complete biocompatibility study according to ISO 10993 takes 3 to 6 months. A prospective clinical investigation can take several years. Identifying this milestone and anticipating it as far in advance as possible is the first priority of sequencing.

Milestone 5: complete technical documentation submitted to the notified body. Formal submission starts the review clock — and the start of certification fees.

Milestone 6: QMS audit by the notified body. Scheduled by the notified body, generally 2 to 4 months after submission of the documentation. Nonconformities raised during the audit generate a compliance lead time before the certificate is issued.

Milestone 7: issuance of the MDR certificate. The final objective.

How to sequence to optimise timelines

The critical path of an MDR certification project almost always runs through the clinical data. All activities that can be conducted in parallel with the clinical studies must be: building the QMS, drafting the GSPR, updating labelling, risk management.

Waiting until the clinical studies are finished to start the QMS adds 6 to 18 months to the total schedule. The QMS can be built independently, then aligned with the clinical documentation when drafting the technical documentation.

The margin to plan for

An MDR certification project without margin is a project that accumulates delays. Requests for additional information from the notified body (inevitable for a first submission), response times from study providers, and internal documentation validation lead times systematically create slippage.

A margin of 20 to 30% on the total estimated duration is a reasonable precaution, not pessimism.
